Users Browsing Site: angelhirschcounseling.com does not require users to complete a registration form or submit any personal information in order to browse this website.

I collect information from you when you request me to contact you, to schedule an appointment with me, or request a recommendation to another professional.  You may be asked to enter your: name, e-mail address, mailing address, phone number, and a brief description of what you seek from counseling. You may, however, visit my site anonymously.

What is your information used for?

I use this information exclusively to connect with you a) as a potential client, or b) to connect you with another professional outside of AHC.

How is your information protected?

I implement a variety of security measures to maintain the safety of your personal information when you complete a request for services.

Do I disclose any information to outside parties?

I do not sell, trade, or otherwise transfer to outside parties your personally identifiable information. This does not include trusted third parties who assist me in operating my website, email system, banking, and electronic health records, so long as those parties are bound by a Business Associate Agreement form.

CMS is authorized to collect the information on this form and any supporting documentation under section 2799B-7 of the Public Health Service Act, as added by section 112 of the No Surprises Act, title I of Division BB of the Consolidated Appropriations Act, 2021 (Pub. L. 116-260). We need the information on the form to process your request to initiate a payment dispute, verify the eligibility of your dispute for the PPDR process, and to determine whether any conflict of interest exists with the independent dispute resolution entity selected to decide your dispute.
