I collect information from you when you request me to contact you, to schedule an appointment with me, or request a recommendation to another professional. You may be asked to enter your: name, e-mail address, mailing address, phone number, and a brief description of what you seek from counseling. You may, however, visit my site anonymously.

I use this information exclusively to connect with you a) as a potential client, or b) to connect you with another professional outside of AHC.
How is your information protected?
I implement a variety of security measures to maintain the safety of your personal information when you complete a request for services.
Do I disclose any information to outside parties?
I do not sell, trade, or otherwise transfer to outside parties your personally identifiable information. This does not include trusted third parties who assist me in operating my website, email system, banking, and electronic health records, so long as those parties are bound by a Business Associate Agreement form.
